From the survays website:

"By doing this short survey of 17 questions, you will help to articulate the needs and experiences of food allergic airline passengers. Please forward the e-mail and survey link to other families and relatives beyond Minnesota since airline passengers live everywhere: more responses will provide a better understanding of flying with food allergies. Survey participants will remain confidential. Survey results may be shared with other nonprofits, the media, or the airline industry."
